NWTA Forms

The forms listed here are for the inevitable and necessary paperwork that is a part of every organization. They are available as downloadable and emminently printable (300 dpi or better) files in PDF format. If you do not have a PDF reader you can download Adobe Acrobat Reader for your machine for free.

• Unit Application Form (97k) - Would your unit like to join our organization? We welcome new units as well as new individual members. Please read our Constitition/Bylaws (above) for details.

• Individual Membership Application and Change of Address Form (16k)

• General Impression Reference Form (16k) - This is the form each recreated unit has on file with the Inspector General that describes and documents the overall background and general appearance of the unit as a whole. If the unit is a uniformed group, most everything necessary for the IIF would be covered here.

• Individual Inspection Form (IIF) (55k) - This is the form, with instructions, that every member needs to complete prior to being inspected. For more information on primary documentation, check out the January '96 Clothing Forum and Backs of Books. The form and inspection seem a bit intimidating, but they are absolutely necessary to maintaining our authenticity, and they really aren't all that bad. • Order forms for the medal for Order of Merit. The Order of Merit is a recognition of individual member participation in multiples of seven years.
